For each referral, please fill out the form and email or fax, along with the patient’s pertinent dermatological records, to our office. Our email and fax number is included on the bottom of the form. It is so important to provide us this background history prior to the patient’s appointment so that Dr. Tapp has all the information needed to help make a complete diagnosis and treat the issue effectively. The client will be referred back to the you, the referring DVM, should they have non-dermatology related questions or concerns.
